In an effort to better serve his patients, Dr. Gaglione has created a Concierge Medical Services Practice. Basically this means that you pay a yearly fee to be a member of the practice and in return you will receive more attention and better treatment than a traditional medical practice. Dr. Gaglione is committed to providing high quality medical services to his patients. Enrollment in Dr. Gaglione’s practice will be limited in order to allow Dr. Gaglione to provide enhanced services to each of his patients..

Why Concierge Medicine?

The philosophy underlying the creation of a concierge practice is that healthcare has greatly changed over the past decades and often not for the better. A significant casualty along the way has been the doctor-patient relationship, which should be an important cornerstone supporting the health and well being of every patient.

In addition to the Concierge Services encompassed by the Patient Fee, the following benefits are provided at no charge because of the nature of a private (or concierge) limited practice size:
• Facilitation of same or next day appointments and fast, friendly access to your Practice physician or nurse practitioner
• Availability of direct telephone and/or email consultations with Dr. Gaglione or another Practice physician or nurse practitioner
• On-time appointments
• Respect for your time
• Dedicated support personnel
• Email and fax access to Practice office personnel
• An unlimited number of appointments during the year
• A tasteful, comfortable and less crowded waiting room
• Unhurried appointments with Dr. Gaglione
• Quick turnaround on laboratory and test results

The trend towards concierge medicine focused on limiting, or eliminating several of the factors which have become barriers to the delivery of excellent, individualized healthcare. Each concierge practice is limited in size, which greatly expands access for each patient to their doctor, allowing for rapid communication and timely visits. Some physicians have always tried to practice this way, but unfortunately the current state of the healthcare system no longer allows for this. With markedly reduced external “non-medical” barriers, the focus can return to what is most important: your relationship with your doctor. In this way, the balance may be restored in favor of providing excellent, comprehensive diagnosis and health management in a comfortable setting.
Most patients who choose to enroll in a concierge medical practice have a long standing relationship with their physician. If you have recognized greater difficulty in access to your physician and felt a decrease in overall efficiency or effectiveness of care, then you will be pleased with the level of care you will receive with a concierge practice.
The ability to have improved access to appointments and communication with your doctor and knowing that your physician is better able to focus on your needs is worth a reasonable financial commitment. In fact most patients who become members of a concierge practice feel that the benefits far outweigh the costs and there are few who decide to return to a “traditional” type practice after they experience it.

Our Concierge Services

By enrolling in the program, you will be entitled to the following “Concierge Services” which are services that are not covered by Medicare and/or insurance policies:
• Annual detailed nutritional evaluation with dietary recommendations by a medical dietitian/nutritionist (not covered by Medicare, Medicaid or by your applicable health plan’s insurance policy if Dr. Gaglione is not a participating PCP with such health plan);
• Annual fitness assessment;
• Annual body composition analysis;
• Annual complimentary diabetes screening, without indication of diabetes (not covered by Medicare, Medicaid or by your applicable health plan’s insurance policy if Dr. Gaglione is not a participating PCP with such health plan)
• Annual comprehensive cardiovascular disease risk assessment and counseling (not covered by Medicare, Medicaid or by your applicable health plan’s insurance policy if Dr. Gaglione is not a participating PCP with such health plan)
• Availability of other services excluded from Medicare coverage pursuant to 42 C.F.R.;
• House calls by Dr. Gaglione, when deemed appropriate by Dr. Gaglione (to the extent not covered by Medicare, Medicaid or by your applicable health plan’s insurance policy if Dr. Gaglione is not a participating PCP with such health plan)
• 24-hour access to Dr. Gaglione and his staff (to the extent not covered by Medicare, Medicaid or by your applicable health plan’s insurance policy if Dr. Gaglione is not a participating PCP with such health plan)
• Availability of other services excluded from Medicare coverage pursuant to 42 C.F.R. § 411.15 and insurance coverage and within the scope of care offered by Dr. Gaglione.
